Louisville, Ky. 3 Sept. 1891 - d. 25 Dec. 1958 From 1922 to 1927 he was musical director at the Dreamland Ballroom in Chicago. In 1926 he received a Ph.D. in music, and henceforth he billed himself as Doc Cook. Born : September 03, 1891 in Louisville, Kentucky. Died : December 25, 1958 in Wurtsboro, New York.
